The forms available below will only need to be filled out once for each student and will cover any activities of the Student Ministry of Lee Road during 2007. The files are in .pdf format, if you need Adobe Reader, get it here for free.

Below are links to forms that should be thoroughly read and signed by students and parents where indicated. Chaperones must also fill out and sign the medical release forms. Please print these forms and bring them to the Student/Parent meeting on Wednesday, January 24, 2007 at 7:00 p.m. in the youth room.
